The Role of Diet in the Mitigation of Alzheimer's Dementia

Risk factors, both genetic and environmental, have been identified for Alzheimer's disease (AD). Epidemiologic studies of dementia and cardiovascular disease show that both of these conditions are prevalent in elderly people and often co-exist. It is possible, then, that lifestyle modifications—such as specific dietary and exercise interventions that have been found to improve cardiovascular risk factors such as hypertension, hyperlipidemia, diabetes, and inflammation (also common risk factors for AD)—may also be useful in mitigating the clinical expression of AD, especially when initiated in mid-life or earlier.

With approval from the Food and Drug Administration (FDA) and a recommendation from the Centers for Disease Control and Prevention's Advisory Committee on Immunization Practices (ACIP) in hand, providers are poised to offer the first vaccine specifically designed to prevent cervical cancer.

The FDA in June approved quadrivalent HPV vaccine (Gardasil, Merck & Co.; Whitehouse Station, NJ). The vaccine targets four types of human papillomaviruses (HPVs): types 6, 11, 16, and 18. HPV types 16 and 18 and others, known as "high-risk" HPV types, may cause abnormal Pap tests and cervical cancer in women, as well as a number of other cancers in the vulva, vagina, anus, or penis.

Finally an Optimal Strategy for Diagnosing Deep Vein Thrombosis

A medical research unit in the United Kingdom analyzed 18 different strategies for managing patients with suspected deep vein thrombosis (DVT). Using systematic review, meta-analysis and cost effectiveness analysis, they settled on an algorithm based on clinical risk. The initial screen for all patients is a latex D-dimer test. If the D-dimer test is negative and the patient is low or intermediate risk (Wells rule), DVT is ruled out. If the clinical risk is high, and the D-dimer test is negative, an above-the-knee ultrasound is ordered, and, if positive, the patient is treated. If the D-dimer test is positive, an ultrasound is ordered regardless of risk. If the ultrasound is negative and the patient is high risk, it is repeated before a treatment or discharge decision is made.
